Competency Framework - Evidence Forms

These are forms to help you document your supported evidence which include; witness statements, testimonies from other colleagues, managers and peers and reflective account forms to help you record achievement of competencies.

Mentees should collate evidence on an ongoing basis and notify their mentor when they that they have sufficient evidence to demonstrate competence at the pre-agreed level (and within agreed time limits).

Competency Framework - Reflective Account Evidence Forms

As with the Core Document, there are two ways to use these forms. Online or Offline to match the type of Core document you are using:

Offline Version: This is a pdf document which you can download and print. You can then work with your mentor face to face to fill out action plans and sign off your evidence.

This version is best if you have limited online access and you can meet with your mentor regularly.

Online Version:

This version is a Google document that you copy to your google account and share it with your mentor. You can then both work on the framework from different locations and it is saved in your google drive.

This means that you don't have to keep track of a physical document and there is always an up-to-date copy somewhere safe. It can be really beneficial if you don't always work in the same location as your mentor but you will need to have a Google account as, before you start
